This is the card that must be filled out completely and turned in to the office prior to playing your first game. Please note the signature spaces on both sides of the card. Cards not completely filled out and signed by the player will not be accepted. We do not "hold" cards for the players to come in and finish. Players must have a completed card turned in and paid for before they are eligible to play. See the Codes of Conduct for more information on how to turn in cards and the season calendar for due dates. Managers, you will receive a packet of 20 cards upon team registration. There is no registration deadline or limit to the number of players per team for our leagues. Limits do apply for state tournaments. Players must be at least 16. Both minors and a parent should sign both sides of the card.
